Herpes Zoster Vaccination and Dementia Risk
For this assignment, I selected the study by Scherrer et al. (2022), which I first came across while working on a dementia project in my previous role. Our team was exploring evidence-based strategies to improve cognitive health in older adults, and this study caught my attention because it examined whether the herpes zoster (shingles) vaccine might also reduce the risk of dementia.
Study Design
The researchers used a retrospective cohort study design. In this type of study, researchers use existing data to compare groups based on an exposure and follow outcomes over time. In this study, patients who received the herpes zoster vaccine were compared with those who were not vaccinated to evaluate differences in dementia risk.
A strength of cohort studies is that they allow researchers to examine relationships between an exposure and an outcome in large populations and establish that the exposure occurred before the outcome. Using existing healthcare data also allows efficient evaluation of long-term outcomes. However, retrospective cohort studies cannot prove causation and may be affected by confounding variables or inaccuracies in medical records (Capili & Anastasi, 2022).
Sampling Method and Selection Bias
The study used a large population-based sample from electronic health records within the Veterans Health Administration. The large sample size improved the reliability of the findings; however, the population may not fully represent all older adults because veterans differ from the general population in demographics, healthcare access, and health risks.
A potential source of selection bias is the healthy user effect. Patients who receive recommended vaccinations may also be more likely to engage in preventive care and maintain healthier behaviors, which could influence dementia outcomes.
Data Collection Methods
The researchers used electronic health records to collect information on vaccination status, dementia diagnoses, and patient characteristics. Using documented clinical data strengthened reliability and reduced recall bias. However, limitations included possible errors in diagnostic coding and incomplete information on lifestyle factors such as exercise, diet, education, and social engagement, which may also affect dementia risk.
Main Findings
The study found that patients who received herpes zoster vaccination had a lower risk of developing dementia compared with those who were not vaccinated. Although the findings do not prove that vaccination prevents dementia, they suggest a possible association and support further research into the relationship between vaccination and cognitive health.
Application to Advanced Practice Nursing
As a future family nurse practitioner, I can apply these findings by incorporating preventive health counseling and evidence-based vaccine recommendations into care for older adults. My previous experience working on dementia initiatives reinforced the importance of identifying interventions that may promote healthy aging.
While herpes zoster vaccination remains primarily recommended for shingles prevention, this evidence provides another opportunity to educate patients about the importance of preventive care. Advanced practice nurses can support patients through vaccine education, shared decision-making, and comprehensive dementia risk reduction strategies, including management of chronic diseases and healthy lifestyle practices.
