I do NOT need a title page.
I need two separate discussion board replies to this SOAP NOTE and I will attach the SOAP note and Transcript. I only need a good paragraph or two per response.
Please answer and address one of the questions in each of the two separate responses:
“Would you make any medications changes for this patient or try a different medication based on the patients history?”
OR answer the question: “Would you treat this patient any different if the patient was an adult instead of an adolescent?”
OR answer the question : “What information is missing from the assessment, if any?”.
