Hello Dr Jones and class
For this week’s discussion, I chose the Robert Wood Johnson Foundation (RWJF) blog, “Ensuring Community Investments Boost Health Equity and Wellbeing.” The blog explains that good health depends on more than medical treatment. It also depends on having access to safe housing, healthy food, transportation, education, and other community resources. The authors point out that healthcare organizations cannot improve health on their own. They need to work together with community organizations and local leaders to meet the needs of the people they serve (Martin & Carey, 2026).
I can relate to this topic because I work in a safety-net hospital where many patients have challenges outside of their medical condition. Some patients cannot afford their medications, some miss appointments because they do not have reliable transportation, and others struggle with housing or food insecurity. These problems make it harder for patients to manage chronic diseases and often lead to repeat hospital visits. The CDC (2024) states that social determinants of health have a major impact on health outcomes and contribute to health disparities.
As a future nurse practitioner, I want to look at the whole patient, not just the diagnosis. In the GI clinic, many patients need long-term treatment, expensive medications, dietary changes, and regular follow-up appointments. If patients do not have the support they need at home or in the community, it becomes much harder for them to stay on track with their treatment. This blog reminded me that part of my role as an NP is to ask about these challenges, connect patients with available resources, and work with social workers and community programs when needed. Even small steps, such as helping a patient find transportation or financial assistance for medications, can make a big difference in their health.
Overall, this blog reminded me that improving health is not only about treating illness. It is also about helping patients get the support they need to stay healthy. As a future NP, I hope to advocate for my patients and work with community partners to improve access to care and reduce health disparities.

Application of Course Knowledge
The Robert Wood Johnson Foundation blog, What Policies Will Allow Immigrants and Their Families to Thrive?, highlights how immigration policies influence health by affecting access to healthcare, education, housing, and economic opportunities. The authors explain that many immigrants face barriers to essential services because of restrictive policies or concerns about their immigration status. These challenges can contribute to health disparities and delay preventive care. The blog emphasizes that inclusive policies promoting equitable access to resources can improve health outcomes for immigrant families and the communities where they live (Robert Wood Johnson Foundation [RWJF], 2024).
This topic resonates with me both personally and professionally because I immigrated to the United States from the Philippines. While every immigrant’s experience is unique, I understand the challenges of adapting to a new culture and navigating unfamiliar systems. Those experiences have shaped my perspective and reinforced the importance of compassionate, culturally responsive care. As a future family nurse practitioner, I will care for patients from diverse backgrounds, many of whom may face language barriers, limited insurance coverage, or difficulty accessing healthcare. In communities like Contra Costa County, these barriers can contribute to delayed care and poorer health outcomes.
This blog reminded me that effective healthcare goes beyond treating illness. In my future practice, I plan to assess patients for social needs such as food insecurity, housing, transportation, insurance coverage, and language access, while connecting them with community resources. My own journey as an immigrant has shown me the value of feeling welcomed and understood, and I hope to provide that same experience for my patients. I also hope to advocate for policies that improve healthcare access and reduce health disparities within underserved communities.
