It should include reflection on the necessary skills for choosing a subject, narrowing your research field, researching effectively, recording accurately and managing time well. You should identify strengths and weaknesses, and include an action plan for overcoming/improving weaknesses. Your Portfolio should be typed and presented in a clear and accurate manner.
