Think back to a critical incident in your workplace that affected patient safety, had unintended
outcomes, or did not contribute to the well-being of the patient. Reflect on the incident to identify
why the breakdowns in quality and safety occurred.
Discuss:

information from your research to identify and talk about the incident and what
worked well and what did not.
1. Example questions to use when analyzing the event
1. Was the nurse confident in their reasoning?
2. Was the whole situation taken into consideration?
3. Was there adequate consideration of alternatives, even those that
were nontraditional or creative?
4. Was the nurse flexible enough?
5. Was the nurse engaged enough to really want to understand fully?
6. Were the nurses decisions based on usual practice or potential
bias?
7. Were there signs in the beginning that there may be an issue?
8. Was everyone’s thoughts, recommendations, suggestions
considered from the beginning?
Your reflection of the incident, what the breakdowns were and why they occurred.
What steps a leader/manager can take to prevent future incidents. Or describe how you
would create an environment where these incidences are less likely to happen. For
example, what would you do differently?
